0.97d12c3 is not compatible with modified hardware, don't use it.
Strange that same chip AT29C512 is OK with 0.98d8 , but fail with 0.98d12c3
Generally 0.98d12c3 and some other 0.98dxxx SW requires usage of 4021 instead of 4014.
I checked AT29C512 and get valid ID with both SW 0.98d8 and 0.98d12c3, I changed 4021 to 4014 in ezoflash4v5 and again both SW returns valid ID. Can't explain.
Do you have programmer logic on sockets or soldered in board ?

Check added IC 74HC04 for connections from 4014 to DB25. Replace chip.
Note, softwares 0.98d2, 0.98d6, 0.98d8, PCB45(0.98d9) don't apply multiple data outputs from 4021(4014).
Software versions 0.98d5, PCB45-1(0.98d9), 0.98d11, 0.98d12, 0.98d12c3 apply multiple data outputs.
